Biopsychosocial Predictors of Sleep Quality in Retirement Home Residents

Objectives: Sleep problems and disorders in older adults are relatively high. They may be seen as primary or as a consequence of another physical or mental illness and the medication use. The aim of this study was to examine the possible association of different biological, psychological and social factors with sleep quality of older adults residing in the institution.
Method: The study is a part of the longitudinal project in retirement homes in Zagreb, Croatia. Participants were 138 residents (78% women) of retirement homes, on average 84.3 years old (SD=6.29), ambulatory, without diagnosis of dementia. Data were collected by a structured interview. Pittsburgh Sleep Quality Index (PSQI) was used to assess the sleep quality (SQ). Variables used to predict SQ were: age, self-reported health, functional ability, cognitive ability, stress, social support, social participation, and life satisfaction.
Results: Total PSQI score was greater than 5 in 70% of participants. Subjective SQ and daytime functioning were relatively high. Sleep medication was used relatively frequently, and sleep efficacy was impaired. Regression analysis showed that the observed predictors significantly explained 26,4% of the SQ variance. Although functional ability, self-reported health and life satisfaction were significantly negatively correlated to PSQI, the only significant individual predictor of SQ was life satisfaction.
Conclusion: The overall SQ of the retirement home residents in Zagreb is expectedly impaired. However, greater overall life satisfaction is associated with better SQ. Our results implicate the importance of support to psychological wellbeing of the retirement home residents.
